Parents Manual & Forms

Welcome to the CCACC Academy 2024 Summer Camp Program! We are anticipating an exciting summer of activities for our students, and you are the very important ones to make it happen. Excellent parents and campers produce excellent programs, and that is our goal.

Mandatory Required Forms

Required for all campers

Please follow the instructions below to submit the mandatory required forms for all campers. Each camper must submit a form separately.

Step 1: Login parent portal; if you do not know your family ID, please type in your email and click “Request my Family ID button”

Step 2: Click CAMP on top of the screen and select “Required Camp Forms & Medical Forms”

 

Step 3: The mandatory agreement to participate form is on the top of this screen. If you haven’t submit the form yet, the form status will be displayed as “Missing”. Please click “View Form” button to fill in all required information. Please sign at the bottom of the screen and click “Submit” button.

Once submitted, the form status column should be updated automatically. 

Optional Forms

NOTE: Students from the State of Maryland DO NOT need to fill out any forms below unless they have an allergy or medical consideration.

Children with Allergies or Medical Considerations: Print & fill out relevant state forms; if medication is required, it must be properly labeled and given to us accompanied by your forms. Lunch is egg and nut free, but the Learning to Cook Camp may include eggs and other ingredients.

International Students & Students from Out of State: Print & fill out the immunization form and other relevant documents.

 

 

Once finished, please scan the form into PDF and image format and upload it to the parent portal
